CERTIFIED NURSE ASSISTANT
CLEANING, LAUNDRY, MEAL PREPARATION, BATHING PATIENTS, SHOPPING FOR PATIENTS, AND TRANSFERRING PATIENTS

NIGHT SUPPORT (ENHANCED CARE SUPPORT)
This service provides non-nursing physical assistance and/or nonphysical assistance during the beneficiary’s normal sleeping hours in the beneficiary’s place of residence. This assistance includes: SLEEP CYCLE SUPPORT • Physical assistance or nonphysical assistance with toileting, transferring, and mobility • Prompting and reminding of medications. The provider can will sleep but must and awaken as identified on the beneficiary’s Attendant Care Worksheet and must provide the beneficiary with a mechanism to gain his or her attention or awaken him or her at anytime. The provider must be ready to call a physician, hospital, or other medical personnel should an emergency arise.

CHORE SERVICES
HEAVY HOUSEWORK INCLUDING – CLEANING INTERIOR SURFACE OF OVENS AND REFRIGERATORS, SHAMPOOING RUGS AND FURNITURE, WASHING THE EXTERIOR SURFACE OF WINDOWS, REMOVING HEAVY ACCUMULATIONS OF DIRT AND GRIME AND ASSITING IN THE REARRANGEMENT AND REMOVAL OF ITEMS WHICH PRESENT A SAFETY OR HEALTH HAZARD FOR OLDER OR DISABLED ADULTS. MOVING/PACKING AND HEALTH/SAFETY.

RESPITE CARE
Respite Care is designed to provide relief for the individual’s family member who serves as a primary care giver. Respite is necessary for families who provide constant care for individuals so family members are able to receive periods of relief for vacations, holidays and scheduled periods of time off. SAME SERVICES AS PROVIDED BY PERSONAL CARE ASSISTANT TO RELIEVE THE REGULAR CAREGIVERS OF THEIR RESPONSIBILITIES FOR A SPECIFIED PERIOD. MEAL PREPARATION; ASSISTING CLIENT WITH THE USE OF THE TELEPHONE; LAUNDRY, AND HOUSEKEEPING.

COMPANION SERVICES
Senior companion service is one-on-one, nonmedical assistance, observation, and supervision provided for a cognitively impaired adult to meet his or her health and welfare needs. The service does not entail hands-on nursing care. The primary focus of senior companion service is supportive supervision. The companion is present to supervise the beneficiary and to assist with incidental care as needed, as opposed to attendant care which is task specific. Companions may also provide leisure activities (for example, read mail, books, and magazines or write letters).

FE- WELLNESS MONITORING
Wellness Monitoring is a process whereby a registered nurse evaluates the level of wellness of a consumer to determine if the consumer is properly using medical health services as recommended by a physician and if the health of the consumer is sufficient to maintain him/her in his/her place of residence without more frequent skilled nursing intervention. Wellness Monitoring includes checking and/or monitoring the following: 1. Orientation to surroundings 2. Skin characteristics 3. Edema 4. Personal hygiene 5. Blood Pressure 6. Respiration 7. Pulse 8. Adjustments to medication.

SENIOR CARE ACT – SELF-DIRECT – PAYROLL AGENT
THE CLIENT IS RESPONSIBLE FOR MAKING CHOICES ABOUT THEIR SERVICES IF THEY CHOOSE SELF-DIRECTED SERVICES. THE CLIENT RECRUITS, HIRES ATTENDANTS OR HOMEMAKERS AND BACKUP WORKERS. WE ONLY ACT AS PAYROLL AGENT FOR THE AIDES WHO PROVIDE THIS SERVICE VIA THE SENIOR CARE ACT PROGRAM.

HCBS-TBI - TRANSITIONAL LIVING SPECIALIST (TLS)
Transitional living services- Services provided by the TLS Specialist which help the individual with a brain injury to learn the skills necessary to be independent. Training in daily living skills may include cooking, bathing, grooming, social skills, and managing medical needs. These services may be provided (based on screening and assessment) up to four hours a day, seven days a week. A person must be receiving transitional living services to remain on the HCBS/HI waiver.
